HOW DO I...
...Register To The Practice?

If you would like to register with us, please ask our reception staff for details. They will explain our policy at the present time. They may then arrange for you to collect any necessary paperwork for completion prior to making an appointment for you to see the doctor for a health check. We may ask you to complete a short questionnaire - this helps us to look after you until your medical records arrive. (You will need to bring a sample of urine with you when you attend.)

...Make An Appointment?

Surgery Hours

The surgery is open Monday to Friday
7.45am - 6.00pm
Thursday
7.45am - 12.30pm

The times that particular doctors are available vary - please ask at reception.

To make an appointment ring 01772 339733/4 between 8.15am - 5.30pm
(we are sorry but we are not able to make appointments outside of these times).

Monday to Friday (Thursday 8.15am - 12.30pm). The staff will try to offer you an appointment to suit your needs.

You can see any of the doctors in the practice. However, it is often best, when possible, to see the doctor who knows you and is familiar with your case. If you wish to see a particular doctor at a particular time try to ring in advance.

Appointments may be up to 10 minutes.

You may be asked to return for a second appointment if you have more than one problem, to ensure the smooth runiing of the surgery or you may be referred to a partner in the practice who has a special interest in your particular problem.

All doctors have a number of appointments available for the same day. These are offered when we open the routine telephone lines (see above) at 8.15am. When all the same day appointments for the doctor of your choice have been used, appointments available for other doctors will be offered.

If you cannot attend for an appointment please let us know, then the appointment can be used by someone else.

Patients who fail to attend for three appointments without explanation are sent a warning letter informing them that they will be removed from our list if they continue to miss appointments.

HOW DO I...
...Obtain A Home Visit?

If you are too ill to come to the surgery ring 01772 335128. Try to request a home visit before 10.30am. The receptionist will ask you for the name, address, phone number and reason for the visit. This information helps the doctors plan their calls according to patients’ needs.

Repeat Prescriptions
If you are receiving regular treatment the doctor may agree to let you have a prescription without seeing you.

You can request a repeat prescription by:
• Calling in person and dropping your repeat prescription slip into the box at reception.
• Writing and enclosing a stamped, addressed envelope.
• Telephoning 01772 626737 Monday to Friday between 9.00am - 12 noon.
• Faxing us on 01772 492248.

Please quote your patient number.

Requests made before 12 noon will be ready for collection after 5.00pm the next working day (requests made on a Wednesday will be ready for collection after 5.00pm on Friday).

After 10.30am all requests for home visits will be passed to the doctor for assesing.

HOW DO I...
Obtain Test Results?

For your test results, please phone 01772 339733 after 11.00am. You will be told if your results are normal or if you need to see the doctor. This usually means the result needs explanation and nothing more serious. To ensure confidentiality your test results will only be given to you.

Hospital Follow Up

Prior to making an appointment to find out the outcome of a hospital attendence, check with us that the letter has arrived. (Please ring between 11.00am and 3.00pm.) It can take a while for letters to arrive - the appointment will be wasted if we do not have the information.
