Emergencies

In the event of an emergency you can contact us at any time on 01772 335128 and arrangements will be made for you to see a doctor. When the surgery is closed there will be a recorded message, so please have a pen and paper ready to take down any telephone numbers.

Forms For Collection

If you leave a form which needs attention from a doctor (holiday cancellations, insurance forms, sick notes etc) it will only be ready for collection after five clear working days. This will be longer for doctors who only work part-time. The receptionist will advise you when it will be ready if you hand it in. We encourage payment to be made by cheque for forms which incur fees (holiday cancellations, insurance forms etc). Please ensure you have completed your part and signed consent where applicable.

Telephone Numbers

Please ensure that we have your current telephone number, (including your mobile number) in case we need to contact you and inform us as soon as possible if you change your address. If your new address is outside our practice area, you will be asked to change your GP.
